We are a Federally Qualified Health Center (FQHC) serving all those in need in the Sacramento area. We have been a level 3 Patient Centered Medical Home since 2011 and were the first to earn that designation in Sacramento. About the Position 

This position has the day to day responsibility for all One Community Health programs, proper referrals to specific departments for insurance screening, family planning services, promotion of sanitation and hygiene screening for communicable diseases aimed at stopping HIV transmission and assisting in retention in care, partner counseling and referral services. Community Outreach through health fairs, health education activities, collecting statistics maintaining records and providing health care referrals when appropriate.

Essential Functions: 

STD Clinic

  • Follow QA requirements of Rapid HIV testing Program
  • Administer Rapid HIV and STD tests during walk-in clinic
  • Counseling about health practices, safe sex practices, STI/HIV prevention,
  • Counseling about prevention tools, condoms, PrEP, PEP, and Treatment as Prevention (TasP).
  • Assessment and prevention/risk reduction
  • Utilize Social Network Testing model to target high risk client partners
  • Follow-up with high risk testing clients during widow period
  • Make appropriate referrals for counseling and other resources
  • Assess client information and contact a member of the mental health team for support when working with mentally ill or intoxicated clients
  • Effectively use supervision to maintain boundaries, problem solves, and provides quality services
  • Keep complete client records in EMR

Community Outreach

  • Work with other community providers to increase Social Network Testing
  • Conduct on-line outreach to high risk individuals
  • Provide HIV/STD testing at community events as assigned (includes working flexible hours in the evenings and weekends)
  • Consistently represent One Community Health's mission and goals to the community
  • Identifies outreach opportunities for the wider community through participation at health fairs, schools, and community based organizations.
  • Referral for screening, nutrition, exercise, smoking cessation, alcohol and drug abuse, domestic violence and sexual abuse
  • Appropriate referrals for high risk population for diabetes, high blood pressure and obesity.
  • Helping individuals and their families understand their medical coverage options and make appropriate referrals.

Family Planning

  • Provides individual counseling to females, male and adolescents regarding anatomy and physiology, human reproduction, contraceptive methods, pregnancy testing, pregnancy options.
  • Administer proper pregnancy test
  • Appropriate referral when need them
  • Protect confidentially of the patient.
  • Comply with all Cares Community Health policies, procedures and employment requirements.

Eligibility Outreach

  • Determine eligibility and facilitate enrollment on Family PACT for eligible patients.
  • Helping individuals and their families understand their medical coverage options.
  • Determine eligibility and enroll in an affordable health insurance options with special emphasis on Covered California Qualified Health Plans
  • Schedule appointments with eligibility workers, and screen for eligibility obtaining required information, such as employment information, income, and other information as needed to complete enrollment during the visit.

Retention in Care

  • Research on EHR (NextGen or Epic) to find more information about patients who are in risk or fall out of care.
  • Weekly report to Retention in Care Lead for data collection and grant report
  • Works closely with retention in care lead, case management on retention and re-engagement in care initiatives.
  • Collaborates with inter-disciplinary team at One Community Health and other community based organizations to facilitate retention in care.
  • Book appointments for new or returning patients into care.
  • Documents on EHR (Epic) all the calls and procedure that RIC member has done to find more information about the patient.
  • Documents on EHR face to face encounters including food vouchers.

Education/Experience Required:

  • High School Diploma or Equivalent required
  • AA degree preferred.
  • Covered CA Certification preferred
  • Family PACT enrollment experience preferred
  • Family Planning Certification preferred
  • STD Knowledge, Medical terminology preferred
  • HIV Test Counselor Certification Training preferred,
  • 2 year experience in HIV prevention
  • Pre-Exposure Prophylaxis, (PrEP) and Post Exposure Prophylaxis (PEP) Treatment as Prevention (TasP) counseling skills preferred
  • Knowledge of basic Primary Care morbidity diseases (Obesity, hypertension, diabetes)
