We are a Federally Qualified Health Center (FQHC) serving all those in need in the Sacramento area. We have been a level 3 Patient Centered Medical Home since 2011 and were the first to earn that designation in Sacramento.

    • We offer a choice between two carriers: Blue Shield and Kaiser Permanente.
    • Blue Shield plans offer $20 PCP/ $35 Specialist co-pay and an upgrade from base network to buy up to enhanced network.

    • Both carriers have $0 Deductible.
    • One Community Health pays about 90% of the Employee only premium and pays 75% of the costs for dependents or partners/spouses.

One Community Health is seeking a Physician Assistant who demonstrates expert clinical judgment and performance in acute and chronic disease management while working in a team based environment. The Physician Assistant will work with other staff members including Nurses, Medical Assistants, Scribes, Laboratory staff, Behavioral Health providers, Social Workers, Clinical Pharmacists, Dieticians, as well as with our Dental team. Two full Saturday and some evening clinics are expected. We are seeking a Physician Assistant who has a specialty in Internal Medicine, Family Medicine and/or Pediatrics with a desire to work with underserved populations.  We are accepting resumes for both full and part time positions.

Education/ Experience Required:

  • Master's Degree required
  • Current license as an NP/ PA.
  • State of California Furnishing License
  • Current CPR Certificate
  • Bilingual (desired)
  • Several years of FQHC or community health center practice experience preferred.
