We are a Federally Qualified Health Center (FQHC) serving all those in need in the Sacramento area. We have been a level 3 Patient Centered Medical Home since 2011 and were the first to earn that designation in Sacramento. About the Position

The Psychiatric Nurse Practitioner is responsible for providing medication supportive services under the Site Medical Director to a designated population of patients with psychiatric disorders. The Psychiatric Nurse Practitioner performs all duties allowed by the Nurse Practice Act for Nurse Practitioners to provide direct patient care through use of standardized procedures and protocols as approved by Cares Community Health.

Essential Functions:

  • Proivde psychiatric, diagnostic, therapeutic and medication services to patients
  • Make diagnosism develop treatment plans and prescribe therapy
  • Consult with social workers, therapists, substance abuse counselors, case managers, primary care medical team, pharmacists and other community agencies as appropriate refarding intergrated patient care
  • Confer with patients family and other support systems regarding illness and treatment
  • Conducts 5150 assessments as needed
  • Appropriately maintain medical records and prepare reports
  • Assist in establishing department programs and policies
  • Maintain familiarity with the Federal and State rukes, regulations, and practices in a Federally Qualified Health Clinic
  • Maintain assinged productivity standards
  • Cultural competency in working with patients with diversity of racial, ethnic, religious and economic status, secual orientation, and gender identity.
  • Other duties as assinged
  • Participates in identification and implementation of systems for incident/problem identification and appropriate resolution and feedback process. Recommend improvements to process in all areas of responsibility. 
  • This position is responsible for reporting suspicion of child, adult(including spouses), elder, and dependent adult abuse as outlined in Penal Code, Section 11166, Section 15630, and Section 1160.

Education/Experience Required:

  • Must be a graduate from an accredited School of Nursing recognized by the California Board of Nursing as a Nurse Practitioner.
  • Three years' experience in a specialized field of practice (Psychiatry).
  • Must have a current California Nurse Practitioner license.
  • American Nursing Credentialing Corporation (ANCC) Certification or American Academy of Nurse Practitioners (AANP) required.
  • Must have a current DEA certificate.
  • Must maintain continuing medical education requirements for licensure.
  • Ability to build rapport with patients.
  • Ability to thrive in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership qualities.
