About the Position 

The Pre-Exposure Prophylaxis (PrEP) Navigator provides intensive care coordination, support, and services to HIV negative individuals who require assistance in accessing and remaining in PrEP care. This individual will coordinate with the Prevention team to initiate access to PrEP care and the Medical and Pharmacy teams to provide assistance to existing PrEP patients. 

Core Duties

  • Implement a client-centered approach with individuals throughout Sacramento County, with a targeted focus on existing One Community Health patients.
  • Assist PrEP clients with services such as, but not limited to, health insurance enrollment, PrEP provider identification, appointment scheduling, management of clinical costs, appointment reminders, and education on the benefits of PrEP and ongoing health maintenance.
  • Empower clients to navigate medical and social systems on their own.
  • Work with Prevention staff and others to identify potential PrEP clients and those in need of PrEP navigation services.
  • Provide support to HIV negative individuals who are interested in starting PrEP and those already utilizing PrEP care.
  • Complete screenings to identify readiness for PrEP and other barriers to care. Work with client to identify position solutions.
  • Assess benefits and insurance, and if needed, provide assistance to access payor programs.
  • Provide PrEP disease prevention and PrEP adherence education and counseling.
  • Effectively collaborate with all individuals involved in PrEP care and program delivery for the purposes of supporting high quality PrEP programming.
  • Make referrals for external services needed to support PrEP care and follow up to ensure service delivery.
  • Provide ongoing follow up to ensure PrEP adherence.
  • In some cases, in consultation with the PrEP coordinator, provide transportation to and from PrEP appointments.

Education and/or Experience Required

  • Bachelor's degree in public health or AA degree in human services related field OR two years' experience working in HIV/STD prevention or other community health programs required.
  • Familiarity with PrEP/PEP and other biomedical interventions.
  • Experience in a nonprofit environment preferred; specific training/experience with HIV and other health related issues preferred.
  • Training/experience working in HIV prevention and health education/health promotion and program development.
  • 2 years of experience providing HIV test counselling and referrals.
  • One year of experience working within a multidisciplinary team, as well as independently
  • One year of experience working with outside providers in collaborative, care-coordination capacity.
  • Knowledge of Ryan White Services is required.
  • Health care setting experience is preferred.
  • Experience in program development, management, financial management and data reporting. Advanced experience with Microsoft software, to include spreadsheets and databases.
  • Must be organized, have demonstrated supervisory responsibilities, and able to work independently while working within a Matrix supervision model.
  • Working knowledge of the health-related disparities faced by the LGBTQ population is preferred.
  • Demonstrated interpersonal skills and ability to engage and work effectively with a diverse staff and patient population required.
  • Ability to travel to all clinic sites and community host sites.
